POLICIES
Notice: The policy statements below are in a condensed format. Further policy details, notices and disclaimers are contained within the Program Agreement which is presented at program sign-up.
Privacy policy - Client confidentiality and privacy principles are utilized to the greatest extent possible in all settings. Individual client sessions offer the greatest degree of privacy and confidentiality. Please be mindful of your contributions during group settings, where other group members are present.
Payment policy, Fees & Amounts- Exalted Health and Wellness does not accept insurance or file insurance claims for services rendered. All payments for wellness improvement services are to be paid in full at the first session, or paid on a pay-per-session basis on each day of a client’s session. It is the client’s responsibility to arrange for and to ensure that valid payments have been rendered. Cash, check, credit cards are accepted. Fees and amounts listed are in United States currency.
Program fees are dependent upon the option chosen:
The fee for each one-hour Wellness Transformation Program (WTP) session is $165.00.
The fee for each one-hour FICS session is $125.00.
The returned check fee is $60.00. Fees and amounts listed are non-negotiable. Fees and amounts are subject to change. Fees and amounts contained within a client’s Program Agreement will remain in effect throughout the duration of Client’s active status in the program.
Appointment Cancellation policy - Appointment cancellations and reschedules not only waste time but create a disruptive “ripple effect” in scheduling. Exalted Health and Wellness cannot guarantee the availability of a reschedule appointment before a client’s next regularly scheduled appointment. To avoid incurring a fee, a cancellation must be TIMELY, and also RECEIVED. To be considered TIMELY, the cancellation must be logged at least 24 hours in advance of appointment time. To be considered RECEIVED, Exalted Health and Wellness must respond back to you that a cancellation message left via phone, voicemail, or email was received. Exceptions to this policy are granted for emergencies, unforeseeable circumstances, weather-related disruptions, and similar. All other cancellations are “late”, and will result in a $25.00 fee, due at next appointment.
Recurrent client cancellations and reschedules will not be tolerated, and typically are a sign of one or more of the following problems:
Attempting to add the new habits and challenges of a wellness transformation program into the client schedule when there is not enough “margin” for it. There must be margin - not only for timely attending scheduled appointments - but also for the addition of new habits and practices into the client schedule. If you suspect you might have too many commitments, you likely have too little margin. Due to the fact that WTP program appointments are designed and scheduled to occur just once every two weeks, recurrent cancellations make it entirely possible to “miss out” on the benefits of the wellness transformation program. From experience, clients who were retired, partially retired, self-employed business owners, students, and all clients who were already maintaining a disciplined schedule did best in the program overall.
Attempting to add the new habits and challenges of a wellness transformation program into the existing client lifestyle, without setting an authentic intention. Without setting an authentic intention to make health and wellness the primary focus, the result can be missed appointments, along with poor follow-through on the “homework” between sessions. Due to the fact that WTP program appointments are designed and scheduled to occur just once every two weeks, recurrent cancellations make it entirely possible to “miss out” on the benefits of the wellness transformation program. From experience, clients who were retired, partially retired, self-employed business owners, students, and all clients who were already maintaining a disciplined schedule did best in the program overall.
Exalted Health and Wellness wants you to succeed! It is best to postpone attempting the program if in doubt about your current level of commitment.
Exalted Health and Wellness will stop a client’s WTP program at client’s request. A written request is needed for record keeping.
Exalted Health and Wellness will suggest stopping (and may discontinue) a client’s program at any time if Exalted Health and Wellness believes that stopping is in the client’s best interest.
Exalted Health and Wellness reserves the right to stop a client’s program at any time if it is in the best interest of Exalted Health and Wellness.
There is NO penalty or fee for stopping under ANY of the above listed circumstances.
Late policy - Starting late or after your scheduled appointment time may result in a shorter client session. This policy is necessary in order to maintain adequate session times for subsequent clients.
